How to make a payment using this form

To pay by credit card, complete the credit card information on this form, print it out and either scan and email it, mail it (address at bottom), or fax it(1-435-259-5823) to us. We'll process it and send you a receipt. You can also call us toll free at 1-800-833-1278 and we'll have you reserved in no time.
To pay by check, complete this "payment form", print two copies, one for your records and one to mail to us with your check enclosed. We will hold your reservation for seven days pending receipt of your check.

Deposit & Refund Policy:

A $15 per person deposit is required within 7 days of making your reservation.
Final payment is due no less than two days prior to your trip departure date.
Cancellations are subject to a $10 per person processing fee if received more than two days before departure. Sorry, cancellations are nonrefundable within 48 hours of your trip departure date and time.
